Polk Scholastic Chess 
Annual Membership Registration Form for 2024 - 2025 Tournament Season

 Entry fees $45.00 per student. Application deadline Sept 21, 2024.   Any applications received after Sept 21 will need to be submitted with a $10.00 late fee (Total $55.00).  Last date for registration with late fees is Oct 1, 2024.

Polk Scholastic Chess is a nonprofit organization offering an extracurricular activity open to all students enrolled in Polk County (Florida) schools, enrolled in Pre-Kindergarten through 12th grade.

If you find that the Coach name assigned to your school is incorrect or not available, please select "Other" and enter the correct information. If your school has not assigned any coach, we encourage parents to sign-up with the school office and register as a parent coach. Home schooled players can be registered by selecting the school that the child is zoned in, based on the home address.

Our volunteers may take photos and videos of players at the tournament and post that on our website and social media. By proceeding with this registration form, you give us that consent and agree with our terms & conditions and privacy policies mentioned on our website and our tournament manual.

Participation Agreement:

Polk Scholastic Chess (PSC) is an invitational extracurricular program affiliated with the School Board of Polk County Schools. All participants (players, coaches, and parents) are subject to the rules and regulations of the School Board of Polk County Schools, and Polk Scholastic Chess, which includes the Code of Conduct and the Volunteers in Polk (VIP) program.

Parents are responsible to ensure that their student(s) are properly supervised. The supervision can be provided by a PSC-approved coach. 

NOTE:

Students that are not properly supervised (all day) will not be allowed to compete in PSC tournaments or rounds. All membership and tournament fees are non-refundable and will be considered a donation to Polk Scholastic Chess if the player decides not to play in all of the tournaments. Please read the Polk Scholastic Chess “Coaches’ and Players’ Manual,” which can be found on the web site at www.polkscholasticchess.com. We will be posting the results of the tournaments on this website and you, as a parent or guardian, have provided your consent to display the name of your child and his/her scores. We will be photographing and utilizing students names for public relations purposes and to publish on to our site at www.polkscholasticchess.com. Providing incorrect information to us could disqualify the player and/or school.  PSC will not be responsible for loss of articles, injuries, etc.
